You insulted him one too many times. You threw the wrong punch. You tried to get him into trouble. You're taking too many drugs or drinking too much alcohol. You stole something that belongs to him, or you did something else.

In general, you messed up. Blake wants to get you into trouble with either the teachers or police. If he really hates your guts, expect some questioning about a stolen item suddenly showing up in your backpack, or some other method of framing. Perhaps he's reporting you out of concern, and doesn't actually dislike or hate you. Success rate will vary, depending on the circumstances.

Before you post your ideas, keep in mind that Blake started to report people at the beginning of junior year and whenever he does report someone, it's almost always anonymous.

Yeah, I can see Blake being concerned about her health, with just how much she’s doing it.

Faith’s going to have to insult Blake for quite a bit of time, or make one that really hurts him, but I can see him attempting to get her into trouble for using drugs or drinking alcohol.
